Vermont School Entry Immunization Requirements

In accordance with Vermont state law, effective July 1, 2012 no student will be allowed admittance to school without one(1) of three(3) following items on record:
- Up to date Immunization Record
- Provisional Letter signed by the primary physician’s office
- Exemption form
Other changes include: exemption forms will need to be filled out annually. Also if a student enters school with a provisional admittance, only six (6) months will be given to complete the required vaccinations.
